Step One: The first thing I did was to sketch out the image, put in the shadowed areas and lay down some base color.
Step Two: I always work on the eyes first.  I figure if I can't get that down, I don't need to go any further so I started refining his right eye.

Step Three: On to the other eye and nose.
Step Four: I started working on the chin and mouth.
Step Five: Forehead.
 
Step Six: I added the lines to his head and then cleaned up his neck.
Step Seven: Ear and some of the lighted area on the neck.
 
Step Eight: I added a soft light to the side of his face and quickly worked up the T-Shirt, making it blend in with the background
Step Nine: When I finished I added a "Noise" filter to add a little bit of a grainy texture to the overall image and cropped it down. You might have to look at the larger version to see the subtleties.
